ER Off-Load Delay Nurses You know, you can say what you like about the U. S. medical system and other countries' socialized medicine is good or bad. I'm not going to have that argument with you here. But, I do like the fact that EMS is part of their healthcare services and are included in decision-making on how healthcare is provided in those areas. In our system, EMS gets under-recognized or unrecognized in looking at how to make the system better. In this article from Canada, they are looking at the city of Greater Sudbury and the fact that they have ambulances stacking up in their emergency department (http://www.northernlife.ca/news/localNews/2012/08/11-ambulance-delays.aspx). Population down, call volume up (http://www.mediccast.com/blog/wp-content/uploads/2012/08/Caring-for-Husband_sm-150x141.jpg)Starting off this weeks news items is an article I found out of Springfield Ohio (http://www.springfieldnewssun.com/news/news/local/ems-runs-up-even-as-population-declines/nP5d2/). It's from the Springfield News-Sun and this newspaper article is looking at the situation whereby their population has decreased in the city of Springfield Ohio by 4700 residents according the most recent census data in the last 10 years and yet their call volume for their EMS ambulance calls have gone up. What is the cause of that? I think that, interestingly enough, this is something that is being questioned by a lot of agencies and organizations around the country and I think that this is something you're going to find in your area as well.   Rural EMT Shortage and Recruitment Meanwhile in Sioux Falls South Dakota, call volume is increasing (http://www.keloland.com/News/NewsDetail6373.cfm?Id=134938) but the volunteers that they have available to run their volunteer ambo service is going down. Those numbers are going down for them and why are these numbers decreasing? They're putting out a call to let people know, "if we have to close the doors and we have to cease ambulance service or only provide part-time ambulance service then the nearest ambulance we're going to get is over half an hour away." This is a good example of educating your public about how your system works and what your needs are. These types of articles are all over the place in rural areas and it's a good example of how you can publicize and provide information to your community about what it is that your EMS service does.   Gun Toting EMTs Finally, I just have a single question for you. How many of you out there carry a gun when you go to work (http://www.news24.com/SouthAfrica/News/Western-Cape-EMS-Medics-shouldnt-use-guns-20120730)? It's certainly unusual here where I live. I don't know of any EMS providers around my area that carry a firearm at work, although maybe they are and I just don't know. But in Cape Town, South Africa, they're looking at their emergency medical services in the Western Cape region and many of them carry guns, firearms to work. The leaders are saying there is not a good reason to be armed on the job as an EMT. Even though their job is a dangerous one, EMT's who carry firearms at work, according to this article, are asking for more trouble from the gun itself being present than as a deterrent or protection from any trouble —————————- Atrial Fibrillation Review Coming up next here on the MedicCast, we're going to jump in and revisit a second tip we've done in the past. Minnesota medics, EMS drug shortages and takeovers and we've got another special interview segment from EMS today 2012, if that’s what you’re looking for, you found it right here on the MedicCast. First Licensed Community Paramedics We'll kickoff the news this week with a segment from Minnesota. In Hennepin County, Minnesota, they are starting a new program with community paramedics, creating the first licensed community paramedics in the country (http://www.kare11.com/news/article/983511/396/Minnesota-first-in-nation-to-certify-community-paramedics). We've talked about community paramedics in the past, talking with Chris Montera and his ground-breaking program out of Western Eagle County, Colorado. We also talked with Gary Wingrove and about the formation and goals of the International Roundtable for Community Paramedicine. EMS Drug Shortages (http://www.mediccast.com/blog/wp-content/uploads/2012/07/Ambulance_lights_sm-150x141.jpg)In the next article from North Carolina, they're talking about EMS medication shortages (http://www.dailytidings.com/apps/pbcs.dll/article?AID=/20120716/NEWS02/207160306/-1/NEWSMAP). These are drug shortages hospitals and EMS agencies are seeing all over the country and I know we are experiencing them here in Maryland. One of the issues we have is trying to find ways to either replace the drugs we have with an alternative medication. While this article points out there is a Fentanyl shortage, Maryland uses morphine for primary pain management and we are substituting Fentanyl for Morphine because of Morphine shortages. In North Carolina, according to this article, paramedics are administering medications that are recently expired in order to provide some care for their patients. EMS Agencies Taken Over In the final article this week, we are seeing quite a few agencies providing EMS services closing their doors, especially in rural environments with low call volumes. In the face of rising fuel prices and other challenges, they were struggling to remain open without the volume to give them a consistent billing resource or profitability to maintain the ambulances. So a lot of communities are struggling with this and in many cases County agencies or local municipalities are taking over the duties to fulfill the needs of these communities. In Johnston County, North Carolina, three EMS agencies were taken over by the county (http://www.theherald-nc.com/2012/07/14/23682/county-takes-over-3-ems-agencies.html), although not without some bumps in the road. Taking over EMS operations is never easy, not even when the crews and vehicles just switch patches and logos. It can be more challenging when one of the agencies is unable to simply turn over capital assets. —————————- Pat Songer Innovates Rural Critical Care Our colleague Chris Montera from the EMS Garage joins us this week as the host of another interview from the EMS 10 Awards at EMS Today 2012 back in March. Pat Songer is the Director of EMS at Humboldt General Hospital in Winnemucca, Nevada (http://www.hghospital.ws/). Pat's innovative management of a rural EMS system that strives to provide care most often associated with larger metropolitan areas. Pat Songer and his team achieved its five-year goal of establishing a critical care paramedic ground system. Over the past five years, Pat has kept to a strict schedule of improvements that has included major ambulance and equipment upgrades and dedicated staff training. As a result, patient transport times have been reduced to an average of two hours, 15 minutes by ground or ground/air combinations, compared to an average of three hours, 30 minutes by helicopter. He is joined by Chris Montera and some of the fellow podcasters from the ProMed Network and they present their product picks for this year and have a discussion on some of the issues that were talked about during the expo. For starters, Natalie Quebodeaux and Jon Cloutier, EMS Marketing Manager for Zoll present a new product called the CPR Dashboard. This product gives the provider real-time CPR compression feed back. It also monitors the depth and rate of compressions and a self-correcting feature that tells the provider whether there is the need to increase pressure or up the pace of compressions. It also has a feature that shows if the CPR is effective and delivers adequate cardiac perfusion. All data are recorded and can be downloaded to another database. Jon also said that Zoll currently has a new product on STEMI management that can transfer ECGs from any device that connects to the internet and those readings can be sent in to any hospital that has internet access. The best part about this? It's free! Next up was Scott Kier with Todd McKinney from IntuBrite. Their product is a light source and ergonomic package for intubating. The major innovation of this product is the use of 2 LEDs that gives off more light and better anatomical view of the airway. The design also gives a good grip and encourages proper positioning into the airway. Dr. Ben Bobrow from the Sudden Cardiac Arrest Foundation came up next and shared his thoughts on the system, public awareness of sudden cardiac arrest and the importance of a system-wide approach to recognition and treatment starting with community based CPR training, Emergency Medical Services system improvements, individual EMT and paramedic code management training, and close integration with hospital based cardiac care services. Chris Montera went ahead and presented his product pick which was the Ferno EZ Glide stair chair. This stair chair can move up a 500 lb patient on a flight of stairs with a push of a button. This can also be used on flat surfaces as well as steep inclinations. It also has IV poles and oxygen cylinder holders for convenience. Natalie Quebodeaux followed with her product pick IPCR. This is an electronic patient care reporting system for iPad which helps eliminate redundant PCR paperwork. With this, you can make copies with one click and it auto-fills which saves time for the provider.
